Creates the nsDocShellLoadState object, which is basically
nsDocShellLoadInfo plus a few extra fields to make it usable as a
single argument to nsDocShell::LoadURI (and eventually
nsDocShell::InternalLoad).
Subframe history handling is a huge logic block in
nsDocShell::LoadURI, which is only used on history loads. This patch
also extracts the logic out into its own function to make the body of
LoadURI clearer.

The main change is to just use the principal bindings pass us to do our
CheckLoadURI check. If we do that, we don't have to care about the current
JSContext.

The JSContext arg of SetHrefWithContext is unused, and the function should have
a different name.
All the current callers of SetHrefWithContext are always called from bindings,
so we can't hit the "no jscontext" cases.
The patch introduces NS_GetURIWithNewRef and NS_GetURIWithNewRef which perform the same function.

This patch adds a check on mDocShell in order to do not throw exceptions when
Location is used with a null mDocShell. This is needed because the location
object is not nullable anymore and it can be used also when the window is not
connected to a docshell.
The idea is to not make consumers think about whether the principal exists or
not when the caller knows for sure that it does.
The substantive changes are in dom/bindings, nsHTMLDocument::SetDesignMode, and
around the CanUseStorage bits. Everything else is pretty mechanical.
