When creating a ReflowInput for inner table frame in
CreateReflowInputForInnerTable(), we pass the customize border & padding
if the table frame is border-collapsed.
Similarly, when creating a SizeComputationInput for inner table frame,
we should be able to pass border & padding rather than having a
specialized logic in SizeComputationInput::InitOffsets(). This patch
makes this a reality.

version causes no regression, and allows us to simplify the code.
It also reverts the test annotations added to bug 1675376 which were

Along with a dependent struct DirectDrawInfo.
This allows nsImageRenderer.h and CanvasRenderingContext2D.h to
avoid including nsLayoutUtils.h.
For nsImageRenderer.h in particular, a forward declaration is
not sufficient as nsImageRenderer stores SurfaceFromElementResult
by value.
A couple of method definitions elsewhere are moved out of line
to keep things compiling without including nsLayoutUtils.h in
additional headers.

Also combine the border and padding arguments for
nsContainerFrame::ComputeSizeWithIntrinsicDimensions(), too. This method
is used as a helper to implement ComputeSize() for various replaced
elements. Its callers are all within nsIFrame's derived classes'
overridden methods, so I'm not bothering to convert them in a separate
patch.

In the next part, I'm going to use ComputeSizeFlags as the arguments in
some ReflowInput's methods. Because nsIFrame.h includes ReflowInput.h,
to solve the circular dependency, ComputeSizeFlags needs to be moved to
somewhere else.
Also, revise the document for ComputeSizeFlag. The rest of the patch is
just dropping `nsIFrame::` and adding `mozilla::` as needed.

In order to apply Automatic content-based minimum sizes, we have to know
the content size on the block axis. We cannot get the content size until
we finish the reflow of the child frames. So we have to keep a flag
which indicates the size of the ratio-dependent axis is overrideen by
aspect-ratio in ReflowInput.
We will set the correct return value in the next patch, For now, we
always return AspectRatioUsage::None.

The inclusions were removed with the following very crude script and the
resulting breakage was fixed up by hand. The manual fixups did either
revert the changes done by the script, replace a generic header with a more
specific one or replace a header with a forward declaration.

At first glance, it might look like this would change behavior, since
FinishReflowChild passes aReflowInput to DidReflow, which in turn
notifies aReflowInput's mPercentBSizeObserver. However, if you examine
how the mPercentBSizeObserver is propagated, it can only be set for the
anonymous block wrapping the children of table cells, the children of
table cells, or additionally a child of a table wrapper frame that has
it set (i.e., a table or its caption, when logically a child of a table
cell). Since all of the frames for which this is being changed are
either internal table elements that are inside of the table, or are
things that can never be a descendant of a table at all, there should be
no change in behavior.

There are two existing callers of IsTableCellFrame that both appear to want to
include BCTableCell frames, but don't. A patch in bug 1547759 will add a
third that wants the same.
The existing users of frame types all have to work around it being a distinct
type, and none appear to want the distinction. This removes that complexity.
If any callers want to make the distinction, they could implement QueryFrame
for BCTableCellFrame and use it. (It's not implemented now, though!)
In a little more detail, prior to this patch (in my debug build, at least):

This is the main performance improvement, and means that we no longer have to iterate all the cells for each column.
It has a couple of behaviour changes:
The first is that we no longer apply stacking context effects (like opacity) to column and column group backgrounds.
I believe this is correct as per both CSS2.1 Appendix E, and css-tables-3 (quoted in nsTableColFrame::BuildDisplayList).
This matches the behaviour of blink and WebKit.
We also previously created items in column,row ordering, whereas now they will be in row,column. In cases where two cells
overlap (using rowspan and colspan to extend multiple neighbours in to the same place) this can render backgrounds in a
different order, but the new behaviour matches blink and WebKit.

This also changes behaviour a bit, previously we interleaved column and column group backgrounds. where we now put all the column group backgrounds behind all columns.
I believe this is the correct ordering as per CSS2.2 Appendix E.
Column backgrounds can overlap when using 'span', and we now render this in a different order, but this matches what other browsers do.

Additionally, this patch makes `nsFrame.h` stop including `nsIPresShell.h`
and makes each users include `mozilla/PresShell.h` instead. So, this improves
rebuild performance of `nsIPresShell.h` (and `mozilla/PresShell.h` in the
future).
Note that due to `nsIFrame::PresShell()`, `mozilla::` prefix is necessary for
`PresShell` in a lot of classes which are derived from `nsIFrame` even in
`.cpp` files.
