Removed GetBaseURL, rename GetMedium to Medium and inline it, remove virtual style resolution methods that forward to style set (callers will call style set methods directly). Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned.
